Dear Mr./Mrs. [Name],

Thank you for your request to register [you] for health insurance.

Unfortunately, we cannot register [you] for health insurance yet

We need a copy of [your] valid residence permit. We will use this copy to check [your] details and determine from which date [your] health insurance can start. Please send us a copy as soon as possible. We will then finalise [your] insurance.

Send us your copy within 4 weeks of the date on this letter

Once we have received it, we will process your application within 5 working days. You can upload your copy quickly and easily via zk.nl/bewijsstuk. Select ‘I would like to register’ and then ‘I have a non European nationality’. Your copy may also be a photo or screenshot. Please make sure that the copy is complete and clearly readable.

The following details must be clearly visible on the document

• Name (initials and surname)
• Date of birth
• Date and place of issue

Please make sure the passport photo on the front and back is blacked out so it cannot be seen.

Is it not possible to send the copy in time?

Then unfortunately your registration will be cancelled. You can then submit a new application. If you have any questions, please do not hesitate to contact us before the submission deadline.
