An employer's statement (pdf).
A Wlz assessment from the Sociale Verzekeringsbank (SVB)*
A payslip
A benefit statement (Unemployment Insurance Act or Sickness Benefits Act)
A posting statement (A1 statement)

* You request an 'assessment of your insurance position under the Wlz scheme' from the SVB. The quick Wlz-check is not valid proof.

What statement do you need?

Are you employed by a Dutch company? Please use our employer's statement format (pdf). It contains all the information we need from you.
Do you receive income from several countries or are you a self-employed person, civil servant or flight attendant? If so, always send us a Wlz statement from the SVB.
